Is Hands-Only CPR An Alternative To The Heimlich?

April 5, 2026

Quick Answer

Hands-Only CPR is not a direct alternative to the Heimlich maneuver for choking emergencies, but it can be used in combination with back blows to aid in dislodging an object from the airway.

Understanding the Differences Between Hands-Only CPR and the Heimlich

Hands-Only CPR and the Heimlich maneuver serve distinct purposes in emergency situations. The Heimlich maneuver is specifically designed to relieve choking caused by an object becoming lodged in the airway, and it should be the primary action taken in such cases.

Techniques for Relieving Choking

If a person is choking, encourage them to perform the Heimlich maneuver on themselves by standing with their back to a stable object and making a fist with one hand. Place the fist above their navel, with their thumb side facing inwards. Grasping their fist with their other hand, they should then push inward and upward with a force of about 5-6 pounds (2.3-2.7 kg) to try and dislodge the object.

Combining Hands-Only CPR with Back Blows

If the Heimlich maneuver is unsuccessful in dislodging the object and the person is unresponsive, start Hands-Only CPR by doing 30 chest compressions at a rate of about 100-120 per minute. After each set of 30 compressions, give two breaths, followed by a 30-second pause to reassess the situation. In addition, five back blows between the person’s shoulder blades, followed by five chest compressions, can also be performed in an attempt to dislodge the object and establish an airway.
