Understanding Burn Classification

Burns are classified into three primary categories based on depth: first-degree, second-degree, and third-degree. A thorough assessment of burn depth is vital in the emergency room or in a survival situation. To assess burn depth, look for the following characteristics: first-degree burns affect only the outermost layer of skin, appear red and swollen, and are painful to the touch. Second-degree burns penetrate deeper, affecting both the epidermis and dermis, and often present with blisters. Third-degree burns, also known as full-thickness burns, destroy both the epidermis and dermis, leaving charred, white, or blackened skin. When assessing burn depth, it’s essential to gently touch the affected area, look for blisters, and examine the skin’s appearance. A burn larger than 3 inches in diameter, covering the palms, soles, face, hands, feet, or genital area, or involving the eyes, ears, or joints, is considered severe and requires immediate medical attention.

Assessing Burn Depth Techniques

To accurately assess burn depth, use the “Rule of Nines” to estimate the affected body surface area, which helps guide treatment decisions. For adults, the head and neck represent 9% of the total body surface area, each arm is 9%, the front and back of the torso are 18% each, and each leg is 18%. Use this information to quickly estimate the severity of the burn. In addition to visual examination, use the “TIMES” method to assess burn depth: T (tan or blackened skin), I (inability to feel sensations), M (moisture or blistering), E (eschar or charring), and S (skin sloughing). This mnemonic device helps ensure a thorough evaluation of the burn injury.
