Mechanisms of Third Degree Burns

Third degree burns are characterized by full-thickness damage to the skin and underlying tissues. They can be caused by various mechanisms, including extreme heat, cold, electricity, and chemicals. Prolonged exposure to cold temperatures can cause frostbite, which can lead to third degree burns if the affected area is not properly treated. For example, temperatures below -20°C (-4°F) can cause frostbite in as little as 30 minutes.

Classification of Burns

Burns are classified into four degrees based on their severity: first degree, second degree, third degree, and fourth degree. Third degree burns are further classified into two subcategories: deep and superficial. Deep third degree burns extend through all layers of the skin and underlying tissues, while superficial third degree burns only affect the epidermis and dermis. Electrical burns, for example, can cause deep third degree burns even with relatively low voltages, as the electrical current can cause extensive tissue damage.

Treatment of Third Degree Burns

Treatment of third degree burns requires immediate medical attention. The affected area should be covered with a sterile dressing to prevent infection, and the patient should be transported to a medical facility as soon as possible. In the field, the burn should be cooled with cool water for no more than 10-15 minutes to prevent further damage. Do not use ice or ice water, as this can cause further cooling and increase the risk of hypothermia. In the medical facility, the burn may be treated with debridement to remove dead tissue, followed by grafting to promote healing.
